    Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences) and our independent living community (Fountaingate Gardens). Our Certified Home Health Agency and Licensed Home Care Agency share an office in Farmingdale, NY.

    The Clinical Nurse Manager is responsible for assisting in the implementation of home health clinical services within the Certified Home Health Agency (CHHA) in a defined area in compliance with applicable laws, regulations and Agency policies. The Clinical Nurse Manager will monitor and evaluate assigned professional and para-professional staff in the local office.

    DUTIES AND RESPONSIBILITIES:

    • Carries out the agency’s mission, philosophy, goals and objectives within guidelines of Agency policy and position function.
    • Interprets and implements the Agency’s philosophy to staff and members of the community.
    • Assists assigned staff to assess patient and family needs, develop plans of care and provide necessary services to families using other disciplines and services.
    • Assists in identifying the needs for evaluation services by others (e.g., physician, social workers,therapists, etc.) and provides administrative supervision of these support services.
    • Assists assigned staff in setting priorities and development of plans or health promotion, preventive, therapeutic and restorative care services to individuals and families, based upon immediate and long range goals.
    • Provides direct services to families where needs may be of such a nature as to require special skills, teaching or evaluation, demonstrates technical skills, or teaching, interviewing, or other abilities to staff or families.
    • Supervises assigned staff including field supervision visits to insure a high quality of professional services and patient care.
    • Provides evaluation of assigned staff which includes teaching and guidance as necessary for professional growth and provision of quality services.
    • Assists in establishing, maintaining, and developing effective working relationships with patient’s family, Agency staff, referring institutions, agencies, physicians and the community.
    • Participates in orientation for new employees: staff, contract and students.
    • Acts as a resource person in the most effective use of volunteers and community resources.
    • Collaborates, implements and supervises special programs which enhance the Agency’s scope of services.
    • Supervise, monitor, and review of all 485s and insurance authorizations to insure Agency reimbursement for services rendered.
    • Supervises and assists in intake referrals to insure that appropriate home care services are provided to clients.
    • Assists in planning and implementing continuing education programs for the development of leadership, professional growth and excellence of patient care.
    • Collaborates and implements a quality improvement program to evaluate care delivered to clients by the Agency staff.
    • Maintains necessary records and reports and utilizes these to interpret health and social needs of the local community.
    • Acts as a resource for and communicates with peers and clinical resource experts to insure that clients receive optimal home care services.
    • Represents the Agency in professional and community health groups.
    • Participates in committees concerned with planning, evaluating, and recommending changes in policies, procedures and programs, including evidence-based clinical care. Committees include but are not limited to: QAPI, Performance Improvement Projects, Performance Advisory Committees, etc.
    • Recommends transfer, promotion and dismissal of staff.
    • Accepts and utilizes supervision in carrying out responsibilities in accordance with professional standards.
    • Participates in office specific responsibilities related to the position.
    • Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.

    QUALIFICATIONS:

    • Current New York State License as a Registered Professional Nurse.
    • Masters Preferred. Minimum of a Bachelor of Science Degree in Nursing, from an approved school is preferred.
    • 3-5 years’ experience as a Community Health Nurse, providing direct nursing services, case management and transitional care with 5 years supervisory experience preferred.
    • Experience with Quality Assurance Performance Improvement (QAPI) and compliance monitoring.
    • Expertise in OASIS D guidelines and education on improving OASIS D assessment
    • Experience in aligning clinical and financial goals for managing all patient care
    • Current driver’s license, required insurance, and care available for work as required.

    KNOWLEDGE, SKILLS AND ABILITIES:

    • Knowledge of the principles and practices of community health nursing and its relationship to other health services.
    • Demonstrated competence in supervision and management of staff
    • Demonstrated competence in clinical skills and procedures required to care for patients and caregivers in their home
    • Exemplary communication and customer services skills
    • Knowledge of geographic area, resources of the community and sensitivity to community needs
    • Ability to establish and maintain effective working relationships with all staff and the community
    • Awareness of development levels of staff and ability to identify problem areas affecting staff development and effectively empowering staff to improve performance and critical thinking skills
    • Ability to set the climate for productive work
    • Knowledge of the value of therapies and consultative services to enhance patient care
