Clinical Supervisor - Home Care supervises and manages home care medical and support staff. Assigns cases and coordinates with physicians, patients, and references. Being a Clinical Supervisor - Home Care trains staff in policies, procedures, and regulations. Onboards and mentors new staff. Additionally, Clinical Supervisor - Home Care requires a bachelor's degree. May require an RN. Typically reports to a manager or head of a unit/department. The Clinical Supervisor - Home Care supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. To be a Clinical Supervisor - Home Care typically requires 3 years experience in the related area as an individual contributor.     Requirements

    A graduate of an approved school of professional nursing who is licensed in Missouri/Compact

    OR a licensed physical therapist, speech‐language pathologist, or occupational therapist

    Minimum of at least two (2) years as a registered professional in the healthcare field, including one (1) year in home care.certifications

    OASIS certified (HCS‐O, COS‐C), preferred.

    Responsibilities

    The Administrator, clinical manager or designated alternate administrator must be available in person or by telephone/electronic means during all operating hours (24 hours per day, 7 days a week)

    Oversight of making patient and personnel assignments

    Coordinating patient care and referrals - Continuous assessment

    Conduct quality assurance documentation reviews of assigned records (specified in interview/orientation)

    Control and monitor schedules of assigned patients and/or clinicians

    Maintain proficiency/knowledge of OASIS Data set items, home care reimbursement and compliance as related to OASIS regulations and requirements

    Develop and maintain knowledge of software systems utilized by the agency

    May be required to attend mandatory scheduled meetings/training

    Participation in quality assurance and performance improvement activities

    Completing home visits and comprehensive/OASIS assessments as needed

    Functions as health care liaison for client, physician and Phoenix Home Care

    Participation in regular on call rotation as directed by the administrator
