Haven is seeking a LPN Clinical Manager to direct, supervise, and evaluate the activities of the clinical area for our Seattle, AZ Office.  The optimal candidate will be a team player, well organized, enjoy mentoring, and strive for growth and excellence. Home Health experience is required.

What You Would be Responsible for:

• Evaluation, oversight, and management of  patients and potential patient care
• QA of all clinical documentation with a thorough understanding of Oasis 
• Excellence in management of PDGM
• Understanding QAPI/Emergency Management/Metric analysis
• Understanding and experience of the Accreditation and survey process to include Medicare COPs, all state and federal guidelines.
• Participation in DON/Metric/Revenue/Educational meetings.
• Team leader of case conferences and staff meetings.

Haven Qualification Standards: 

• Must be a graduate of an approved school of nursing
• Must be an RN or LPN 
• Currently licensed in the state of the HHA

• Excellent knowledge of nursing care methods and procedures

• A team player with excellent communication and interpersonal skills
• Responsible and compassionate
• Outstanding organizational and multi-tasking skills
• Patient with excellent problem-solving skills
• At least two (2) years of Home Care experience with at least one (1) year supervisory preferred
