Job Summary: The Director of Quality and Population Health works in collaboration with all senior and middle managers (clinical, fiscal, community based, and operations) to provide leadership for development and implementation of Quality Management initiatives that align with the organization’s strategic mission and vision. Within this collaborative approach, oversees the daily administrative, and operations activities of the Quality and Population Health Department. Supervises the Population Health Department. This is an exempt position.
Required Skills/Abilities:

Knowledge of quality improvement tools and techniques.

Experienced in the use of data analytics software tools.

Proficient in Microsoft Office 365 tools; knowledge of Power BI a plus.

Knowledge of medical and dental electronic medical records software.
-
Ability to organize, prioritize and problem solve independently.

Excellent organizational, communications, written and customer service skills.

The ability to work well with people at all levels.
Education and Experience:
-
Master’s degree in health care administration, public administration, public health, nursing, or related field.

Five or more years in a quality assurance or quality control position in a health care setting.

Experience in Primary Care; FQHC experience a plus.

Clinical experience a plus, but not necessary.

Duties/Responsibilities:
-
Participates in collaboration with community partners to ensure the health of the communities served by CCHS.

Participates in grant writing process to support new practice transformation initiatives.

Serves as the quality lead for Medicaid managed care organizations, Medicare Advantage plans and private payers including HEDIS documentation requirements and incentive programs.

Working closely with the Senior Management Team, serves as the organizational lead for Maryland Primary Care Program (MDPCP).

Directs the activities of the Population Health Department, including care coordination, care navigation and outreach, ensuring compliance with MDPCP regulations as well as Medicaid MCO contracts.

Collaborates with the Chief Clinical Officer and Medical Clinical Care Director on Practice Transformation activities utilizing care coordination, care navigation and outreach.

Collaborates with the Clinical Application Specialist, EMR/EDR Optimization Managers, and operations/clinical directors to ensure appropriate use of the electronic Medical Record (EMR) and Electronic Dental Record (EDR) for collection and use of medical and dental clinical data for population management and clinical quality improvement.

Maintains a detailed understanding and working knowledge of clinical information systems, their relationship to other systems, and their relative value to clinical operations.

Collaborates with Leadership staff on organizational strategic goals and data collection/measurement methods.

Collaborates with the Chief Quality and Compliance Officer to identify patient safety issues and risk management trends.

Facilitates clinical quality data analysis and communication to appropriate parties, including other leaders and providers.

Produces reports of clinical and operational data needed for submission to government agencies (UDS, CMS, HRSA) and for use by senior management team for strategic planning and operations (340B and Pillar Goals).

Effectively leads staff by developing cohesive teams, mentoring, and coaching.

Identifies staffing needs for the Quality Management Department, participates in the HR hiring process and orients new staff to their roles and responsibilities.

Performs annual assessments of staff including 30-, 60-, and 90-days check-in for new employees.

Ensure staff rounding with all employees supervised within designated timeframes.

Leads Quality Management department meetings and ensures timely communication of information.

Participates in the Organizational Quality Improvement Committee and Safety/Infection Control Committees.

Regular, reliable attendance is a requirement of this job.
