Community Health Director is responsible for developing plans for community-based health programs aimed at the prevention of disease and promotion of health. Recommends community health initiatives, policy implications and best practices after reviewing health literature and statistics. Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.       Job Summary:

         


    Focuses on improving access to health   care services, increasing health and screening, enhancing communication   between community members and health providers, and reducing the need for   emergency and specialty services. Coordinate/participate in community events,   meetings, and other agency and community activities to promote and increase the   usage of Care Alliance Health Center. Provides a variety of specialized services to   special populations. The CHW is expected to understand mental illness and   addiction, being willing and ready to engage with clients who struggle with   these issues at all levels. 

    Works with the community at the grassroots level to promote health access via Care Alliance Health Center (CAHC). The CCHW builds relationships with community members and front line staff at local institutions including schools, social service agencies, businesses, shelters, and places of worship to create pipelines for new CAHC patients and inform CAHC’s responsiveness to the community through our programs and services.


     Responsibilities:

    • Establish positive relationships with front line staff at identified organizations, Flag potential relationships for CAHC leadership
      Work with vulnerable populations with chronic illnesses, mental illness, and substance abuse; individuals who are homeless or living in public housing
    • Represent and promote CAHC at relevant community events such as CMHA resident meetings, parent meetings
      Educate community member and leaders on available clinical and supportive services at CAHC, targeting messages to population. Link potential patients to services, including scheduling appointments.
    • Conduct community presentations and workshops on CAHC services and health topics
    • Design and implement community events such as open houses and health fairs
    • Plan and execute grassroots community outreach events.
    • Serve as a liaison between the community and CAHC, providing relevant feedback and informing operations and programs, and coordinating the use of community spaces within CAHC facilities
    • Support community engagement in the delivery of services at CAHC through the Community Advisory Boards, focus groups, and patient satisfaction surveys
    • Organize activities to meet CAHC priorities and goals

    Requirements

    Required:

    • Associate degree in Social Work, a related field or commensurate experience.  Bachelor's degree preferred.
    • Experience working with a FQHC or community health clinic preferred
    • Ability to work outside, sometimes in inclement weather
    • Minimum of 2 years community engagement experience, working with vulnerable populations with chronic illnesses, mental illness, and substance abuse; individuals who are homeless or living in public housing
    • Ability to quickly assess and respond to requests for support·
    • Strong problem-solving skills· Ability to plan, organize and complete associated paperwork in timely manner and maintain confidentiality
    • CHW Certification or willingness to obtain certification within 6 months
    • A valid driver’s license,
    • Some evening and weekend hours are required

    Technology Requirements:

    Basic understanding and familiarity with the following:

    • Proficient in Microsoft Office, Word and Excel
    • Microsoft Outlook for emails
    • Experience using an EHR.  EPIC experience is a plus
