Exciting opportunity to join a team of committed professionals working to improve perinatal and infant health outcomes in Chautauqua County!

The Perinatal/Infant Community Health Worker (CHW) is responsible for helping families to navigate and access community services and adopt healthy behaviors to improve maternal and infant health outcomes. The CHW home-visiting services to approximately 25 clients at any given time. The CHW conducts routine home visits, individual and family assessments, and provides basic health information and community referrals. The CHW focuses on individual level strategies in Chautauqua County where impact will be the greatest to improve perinatal and infant health outcomes.

REQUIREMENTS/QUALIFICATIONS:

• Associates degree in health and human services or related field or HS/GED diploma with 3 years related experience

• Experience in service coordination and/or assisting clients in improving healthy behaviors

• Reliable transportation for home visiting

• Friendly, compassionate, and empathetic personality

• Ability to document effectively and use data entry system

• Able to work well independently and with others

• Able to manage time and schedules effectively

• Strong communication and organizational skills

• Chautauqua county resident

• Writing ability sufficient to provide adequate documentation in the family record, referral forms and other service coordination forms, and reading ability to the level necessary to comprehend training materials and assist others to fill out forms

Schedule: Potential ability to work some remote hours; Standard work week: Monday – Thursday (8:00-4:30); Friday (8:30-1:00) but may have non-traditional hours.

Job Type: Full-time
