Community Health Director is responsible for developing plans for community-based health programs aimed at the prevention of disease and promotion of health. Recommends community health initiatives, policy implications and best practices after reviewing health literature and statistics. Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.   • Job scope: The Community Health Worker (CHW) Program Manager position provides oversight authority over the CHW Program. The CHW Program Director will spend, at a minimum, fifty percent (50%) of work time delivering direct participant services that meet performance measures. Community Health Worker is key in the role of increasing opportunities for substance users, including opioid users, to reduce any type of harm related to users' substance use. The CHW will address the following: increase retention of eligible population in substance use and mental health services, help eligible population address any medical needs developed from their substance use disorder (SUD), help eligible population eliminate their substance use disorders (SUDs) and build a foundation for their recovery. The CHW will work with youth and adults who have substance use issues. The CHW addresses health disparities that adversely affect groups of people who have systematically experienced greater obstacles to health based on their racial or ethnic group; religion; socioeconomic status; gender; age; mental health; cognitive, sensory, or physical disability; sexual orientation or gender identity; geographic location; or other characteristics historically linked to discrimination or exclusion."


    Education/licensure:
    Prefer Bachelor's degree in psychology, social work or other substance use related fields; Licensed or Certified as Recovery Coach, Peer Provider, Licensed Chemical Dependency Counselor (LCDC), Registered Nurse, or Licensed Vocational Nurse. Will obtain and maintain Texas Department of State Health Services (DSHS) CHW certification. Valid driver's license in state of residence and ability to be insured.


    Experience/Knowledge:
    Must have a minimum of two (2) years of experience in one or more: substance use outreach, substance use intervention; or substance use treatment. Must have a minimum of one (1) year experience in at least two (2) of the following: Working with prison populations, working with individuals with housing instability, working with individuals with SUD, Human Immunodeficiency Virus/Sexual Transmitted Diseases (HIV/STDs), and behavioral health issues, Community health work, or supervisory experience. Knowledge of Motivational Interviewing techniques, service coordination, and crisis management and prevention.


    Physical requirements that are bona fide occupational qualifications essential to the performance of the job:
    Ability and strength to sit, bend, and reach during an 8 to 10 hour day. Able to lift a minimum of 10 pounds. Must be able to travel by automobile for up to 6 hours daily. Sensory acuity sufficient to evaluate, treat, and monitor patient needs and maintain accurate records. Fine motor skills sufficient to legibly document and communicate.
