Community Health Director is responsible for developing plans for community-based health programs aimed at the prevention of disease and promotion of health. Recommends community health initiatives, policy implications and best practices after reviewing health literature and statistics. Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking a part-time Community Health Educator. 

    Primary Responsibilities: 

    • Under the direction of the Manager of Nutrition Education the Community Health Educator is responsible for increasing healthy living behaviors by providing health education to members in the HUSKY Health Program as well as various community audiences.

    Tasks Performed: 

    • Travels state-wide, teaching culturally appropriate health and nutrition information in small-group settings and at community health and wellness events. 
    • Works a combination of weekdays and weekends - mornings, afternoons, and into the evening.
    • Does research, webinars, attends meetings, and other projects as assigned. 
    • Administer and collect pre/post assessment during nutritional workshops. 
    • Ensure data entry of surveys within established guidelines. 
    • Cultivates and maintains awareness of community cultures and values. 
    • Works collaboratively with community engagement team in coordinating partnerships and opportunities with community organizations.

    Essential Functions: 

    • Provides education and resources, to all ages, on nutrition topics and health conditions such as, obesity, hypertension, heart disease, stroke, diabetes, etc.
    • Sends referrals to the Community Engagement Coordinator to assist members in appropriately utilizing the HUSKY Health program. 
    • Participates in health fairs, group workshops, meetings, webinars, health promotion, and outreach activities online and in the community. 
    • Performs other related functions as assigned.
    • Travels across the entire state of CT and agrees to mileage reimbursement policy. 
    • Completes necessary tasks before and after events, including printing/scanning materials, picking up/dropping off supplies, and office work.
    • Checks email at least twice daily and responds as needed. 
    • Works all hours allotted per pay period.

    Desired Education: Four years post-secondary schooling

    Desired Degree: Bachelor’s degree required

    Desired Major: Health services, allied health, nutrition or public health discipline

    Desired Job Experience: One to three years' relevant experience. Experience in health education, with knowledge of Medicaid population preferred. Experience working in a community-based setting for at least 1-2 years preferred. Bilingual - English/Spanish speaking strongly preferred. 

    Certifications Required: State motor vehicle license
