Established in 1958 as the North Essex Child Guidance Clinic, Northwest Essex Community Healthcare Network, Inc. (NECHN) began serving children with serious emotional disturbance and their families. Since that time, we have grown into an agency offering a comprehensive range of services, which include Outpatient Treatment, Co-Occurring Treatment, Addiction Services, Case Management, Crisis Services, and Partial Care. Northwest Essex is also one of seven “Certified Community Behavioral Health Clinics” (CCBHC) selected by the N.J. Division of Mental Health and Addiction Services.

POSITION SUMMARY: This position provides Case Management Services to youth and young adults to help them gain access to resources and services needed to meet their mental health needs, achieve recovery, and live independently in the community. services are coordinated with internal and external providers to help each consumer achieve personalized goals.

Qualifications

• BA in Human Service field (Preferred)
• CHW certificate or ability to obtain within 6-9 months days from start date
• Demonstrates strong communication, leadership and interpersonal skills
• At least 1 year experience in working with youth and young adults
• Attend health/educational fairs to provide educational materials and information about the agency and our funding sources
• Provide culturally competent care to a diverse population of youth/young adults and families
• Act as an advocate and liaison between the youth/young adult/families and community service agencies/schools
• Educate guardians/parents about behavioral health and substance use disorders to help destigmatize the same and to offer knowledge and guidance about appropriate responses to youth/young adult behaviors
• Help to educate families about school services such as Child Study Teams and provide informative workshops. Issue pre/post tests/surveys for outcome measure purposes.
• Work closely with schools and healthcare providers to ensure persons' served have comprehensive and coordinated care plans
• Help youth/young adults/families set personal, educational and health related goals
• Assist youth/young adults and families with program enrollment such as Medicaid services
• Provide advocacy, education and outreach on behalf of vulnerable youth and young adults
• Ability to work with computers and necessary software including Office 365 and Electronic Health Records
• State Screening Certification (Preferred)
