Community Health Director is responsible for developing plans for community-based health programs aimed at the prevention of disease and promotion of health. Recommends community health initiatives, policy implications and best practices after reviewing health literature and statistics. Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function. Job Description:
Works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care.
Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness.
Provides education and behavior modification strategies and provides social support to patients and families.
Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.
Job Requirements:
Education: High School Grad or Equivalent
Licensure: Current and valid Drivers License with an acceptable driving record required.
Experience: Experience working in a community-based setting preferred. Knowledge of medical terminology preferred. Bilingual in English and Spanish is preferred.
