Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. Job Summary
The Senior Community Health Advisor (SCHA) educates potential PACE participants about the PACE program, and assists eligible Seniors, their families and/or designated caregivers through the enrollment process. The SCHA is an ambassador in the community and works alongside the Business Development Department to support a team of Community Health Advisors to promote the PACE program within target communities. They also train Community Health Advisors to provide education and assist in introducing PACE services to eligible seniors and set team goals.
