Recommends community health initiatives, policy implications and best practices after reviewing health literature and statistics. Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     Job Summary
    Works within the community providing outreach to patients with complex medical and social needs. Acts as a resource to assist patients in navigating the health care system.
    Job Duties
    • ​Assists in the identification and outreach of medically complex patients.
    • Collaborates with the multidisciplinary team in the development and implementation of the care plan.
    • Engages patients by way of home visit, hospital visit and accompanying patients to Providers appointments as needed.
    • Completes Vital Signs
    • Identifies gaps in care and connects members with resources
    • Facilitates telemedicine visits with members and providers
    • Performs disease specific education
    • Conducts safety evaluations in patients home
    • Communicates all concerns to clinical team and manager as necessary, seeking support and guidance proactively.
    • Provides reinforcement of an established treatment plan.
    • Builds relationships within the community in order to identify resources that are available to the patients i.e.: DME vendors, Home Health Agencies, Area Aging Office etc.


    Work is typically performed in an office environment. Accountable for satisfying all job specific obligations and complying with all organization policies and procedures.     Position Details

    Hours: 8 AM to 4:30 PM (4-days per week) and 9 AM to 6:00 PM (1-day a week)

    Travel: more than 50%

    Education: High School Diploma or GED is required

    Experience: 3 years related work experience is required
