Community Health Director is responsible for developing plans for community-based health programs aimed at the prevention of disease and promotion of health. Recommends community health initiatives, policy implications and best practices after reviewing health literature and statistics. Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function. Position Overview
A Community Health Worker is a front line public health worker who is a trusted member of and/ or has unusually close understanding of the community served. This trusting relationship enables the HCW-PC to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
A CHW-PC also builds individual and community capacity by increasing the knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
Location and Hours of Position
PHN New Castle PC and Ellwood City HC. The general office hours for this position is 8:30 AM-5:00 PM. The position is located in Lawrence, PA.
Compensation
Minimum salary of $36,000 per year. PHN recognizes experience, education and job location when determining wages.
