Our healthcare facility is hiring a Community Health Worker to join their team! Working 2 days in Silsbee and 3 days in Jasper each week, the Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. In addition, the CHW will provide social support and informal counseling, advocate for individuals and community health needs, and provide related services.
