    Position Description:
    This is an internal recruitment. Only active University Corporation employees will be recruited.

    DEFINITION: The full-time, benefited position is dependent upon grant funds, which expire on June 30, 2022. In the interim, there will be multiple concerted efforts to seek additional funding. PURPOSE: The Outreach Specialist's primary function is to coordinate and facilitate street outreach and field-based case management activities to assist people in finding and implementing solutions to housing barriers, maintaining healthy and safe living environments, and achieving wellness. In addition providing guidance, instruction, direction, and leadership to the outreach team to achieve a key goal is desirable. POSITION SUMMARY:The Outreach Specialist is part of the Homeless Engagement and Response Team (HEART) and will strategize, plan, and implement targeted searches for homeless individuals and encampments identified by the Coalition of Services Providers. Furthermore, the Outreach Specialist will lead a small team of workers to conduct outreach and case management activities for people experiencing homelessness in Monterey County. This position is under the CHE Principal Investigator's oversight and the CHE Senior Manager's general supervision. ESSENTIAL DUTIES AND RESPONSIBILITIES include, but are not limited to, the following:
    • Strategize, plan, and implement targeted searches for homeless individuals and encampments identified by the Coalition of Services Providers
    • Lead a small team of workers to conduct outreach and case management activities.
    • Conduct daily county-wide outreach and case management activities to develop relationships and build rapport with clients to assist them with finding a traditional home (apartment, house, room for rent, etc.) within the community of their choice. Services may include but are not limited to assisting the individual with gathering documentation, accessing appropriate mental and physical health services, and any other services that may help an individual improve their quality of life as they define it.
    • Conduct health and mental health assessments and referrals to appropriate service providers for treatment.
    • Provide supportive counseling to individuals regarding the use and effect of alcohol and drugs.
    • Assist individuals in identifying their need for treatment, provide substance use treatment referrals, and collaborate for individualized care
    • Provide assistance and linkage to housing referrals and health resources.
    • Meet with established clients to review, evaluate, and support goal completion.
    • Provide a high-quality customer service environment for all clients, community partners, and collaborating agencies.
    • Communicate and collaborate respectfully and well with multi-disciplinary team members
    • Respond within specific contract timeframes to requests for count-wide outreach services and send follow-up information to the requestor and the supervisor.
    • Collaborate with community substance abuse, health, and mental health providers to ensure seamless referral and secure connection with services.
    • Work with local law enforcement, interim service, and Veteran service providers to ensure a smooth transition from street living to temporary housing or the permanent home of their choice.
    • Provide ongoing information, referrals, linkages, and advocacy for all other identified needs.
    • Creatively use and develop community resources to broker and link clients to services.
    • Conduct CARS/ VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) on all new clients to connect them to proper intervention and services. Document results, and complete match initiation form.
    • Maintain documentation standards as set forth by CHE policies.
    • Ensure all program data is accurate and entered into the appropriate program documentation system as contractually required. (i.e., HMIS, internal spreadsheet, CARS, etc.)
    • Generate client data for reporting. Prepare case-related reports, including but not limited to demographics, outcomes, successes, and challenges.
    • Must have own transportation, valid driver's license, auto insurance to conduct job-related travel, and the ability to complete web-based training in defensive driving.

    • Performs other job-related duties and special projects as assigned

    Physical Work Environment
    Ability to: operate a motor vehicle; climb stairs; possess good communication skills; meet the classification requirements, and have mobility, vision, hearing, and dexterity levels appropriate to the duties. Capable of the following extended activities: standing, sitting, climbing stairs. Capable of the following intermittent activities: lifting 25 pounds or more and bending in the performance of infrequently performed office duties.

    • Availability to work evenings and weekends as requested by management and based on the operational needs of the Street Outreach program
    • Strong written and verbal communication skills
    • Ability to work and communicate as part of a team
    • Ability to maintain regular and consistent attendance
    • Ability to obtain CPR/First Aid certification
    • Knowledge familiarity with Person-Centered and Solution-based approaches, Motivational Interviewing, Harm Reduction, and Housing First models of service delivery



    Minimum Qualifications:
    • A degree in health and human services is preferred, or enrollment in an accredited institution is preferred.
    • Minimum of two years experience in providing guidance, instruction, direction, and leadership to a group of individuals to achieve a key goal is desirable.
    • Bi-lingual capabilities are helpful but not required
    • Demonstrated understanding of and commitment to CSUMBs vision statement.

Classification: Grade 3- Staff. The hourly rate for this position is $27. This is a non-exempt position and as such is eligible for the overtime provisions of the Fair Labor Standards Act.
