• The Clinical Coordinator is responsible for the coordination, continuity and quality of clinical care following a physician approved plan of treatment.  
• The Clinical Coordinator facilitates the completion of documentation to comply with regulatory, operational, and financial requirements.  
• The Clinical Coordinator acts as a clinical resource to internal and external customers. 
• Has responsibility for managing associate performance, development and coaching of assigned clinical associates.

Graduate from an accredited school of nursing

Licensed RN in Minnesota

Two years RN experience with one year in Home Care

Two years management or supervisory experience in a healthcare setting, preferably Home Care

Preferred

BSN degree

Hospice experience
