Care Coordinator
Duties:
- Provide care and support to individuals with developmental disabilities, dementia, or other special needs
- Assist with activities of daily living, including bathing, dressing, and grooming
- Administer medications according to prescribed schedules
- Monitor vital signs and report any changes or concerns to healthcare professionals
- Assist with behavior management techniques and strategies
- Coordinate and schedule appointments with healthcare providers
- Maintain accurate and up-to-date documentation of care provided
- Collaborate with other healthcare professionals to ensure comprehensive care for individuals
- Provide emotional support and companionship to individuals under your care
Qualifications:
- Previous experience in caregiving or related field preferred
- Knowledge of medication administration and basic medical procedures
- Understanding of behavior management techniques for individuals with developmental disabilities or dementia
- Ability to work effectively with individuals of all ages and backgrounds
- Excellent communication and interpersonal skills
- Compassionate and patient demeanor
- Ability to work independently and as part of a team
- High school diploma or equivalent required
