PURPOSE:
The Personal Care Coordinator is responsible for the coordination of services between clients and caregivers, for the implementation of schedules, adequate staffing and continuity of care to best meet the needs of the clients. The Coordinator reports directly to: Director of Personal Care and/or Administrator
Responsibilities
01. Schedule appropriate disciplines for hours of service on new clients and open hours/shifts on existing cases. Provide consistency in caregiver scheduling with clients.
02. Maintain computer schedules and ensure timely data entry for clients and caregivers. Contact clients and caregivers regarding day-to-day scheduling changes.
03. Reconcile the schedule with Personal Care Attendants Service Delivery Records, making appropriate corrections to the system to facilitate accurate processing for payroll and billing
04. Provide regular updates to the Director of Personal Care and/or Administrator on any open shifts an exhaustive effort has been made to staff the shifts.
05.Provide timely reporting to Director of Personal Care and/or Administrator on caregiver supply and demand
06. Communicate and reinforce Tech Medical’s policies and procedures.
07. Communicate and refer appropriate matters to the Director of Personal Care and/or Administrator for direction
08. Perform general office duties including but not limited to word processing, filing, reception and telephone services.
09. Maintain positive relationships with all clients and their families, prospective clients, caregivers, prospective employees, fellow team members and referral sources.
10.Maintain absolute confidentiality of all information pertaining to employees, clients, clients families and Tech Medical.
11. Conduct Supervisory visits to ensure Personal Care Attendants are completing tasks and clients are 100% satisfied.
12. Demonstrate dependability and reliability.
13. Maintain professionalism, provides support and encouragement to the caregiver team.
14. Rotate on-call weekly as directed. Keep on-call materials current with client information, instructions and directions, current roster of care and roster of active direct care staff.
15. Respond within 30 minutes to voicemails and texts when on-call.
16. Perform other functions as deemed appropriate by the management team.
17. Other duties as assigned by Personal Care Director and/or Administrator
