Triza Home Health Care has a part-time and full-time employment opportunity available as an Intake Coordinator in our Fremont office. Office hours are Mon-Fri 8:30am - 5pm, on-call outside of those days and hours.
Description: The role is responsible for the referral and patient intake process including data entry, establishing and maintaining positive relationships, professional communication, and coordination with patients and their families; maintaining positive working relationships with current and potential referral sources; responding to customer requests and concerns; and patient insurance verification and authorization processes. This position will also require monitoring portals for referrals, adding patients to our EMR system, and will perform other duties as assigned. You will also help gather, collate, and report referral statistics including key customer referral trends. In addition, you will help ensure that all our patients at Triza Home Health Care are receiving great care with the help of the team.
