As an LPN Intake and Scheduling Coordinator, your responsibilities will include;

• Acts as a liaison between field staff, hospital discharge planners, insurance company reps and office staff (20%).
• Providing administrative support to Clinical Management staff (10%).
• Being responsible for intake, staffing and following up of routine referrals, client’s file maintenance and management (70%).
• Performing PPD'S and hosts nursing orientations and performs skills checks for HHA.
• Verifying insurance information on referrals and maintaining logbook.
• Screening files for deficiency and any other issues and following up with corrections.
• Creating appropriate logs to monitor performance improvement.
• Maintaining schedules and geographic coverage for staff.
• Making HHC-3000 data entries.
• Ensuring information is communicated to field staff, office staff, and customers in a timely manner.
• Participate in on call duties.
• Performing specific tasks as assigned.

What you will need:

• Current Active PA LPN License required.
• Experience in home health or an agency environment highly desirable.
• Exposure to best practices in the fields of pediatric, geriatric, and home health services.

Job Type: Full-time
Salary: $55,000.00 - $65,000.00 per year
