    Job Description: We are currently seeking a skilled and dedicated Licensed Practical Nurse (LPN) Case Manager to join our team at Interim HealthCare Muncy Office. As an LPN Case Manager, you will play a critical role in auditing, conducting client visits, and providing skilled nursing care while supervising other healthcare professionals.     Responsibilities:

    • Conduct audits of patient records and clinical documentation to ensure compliance with regulatory standards and organizational policies.
    • Perform client visits to assess their health status, develop care plans, and provide skilled nursing care as needed.
    • Supervise and support skilled employees, including Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs), ensuring adherence to care plans and quality standards.
    • Coordinate with healthcare providers and interdisciplinary teams to ensure comprehensive and coordinated care for clients.
    • Provide hands-on nursing care, including medication administration, wound care, and other clinical procedures, as needed.
    • Document all client interactions, assessments, and interventions accurately and in a timely manner.
    • Participate in case conferences, team meetings, and quality improvement initiatives to enhance the delivery of care.
    • Act as a resource for staff regarding clinical procedures, protocols, and policies.
    • Assist in the training and onboarding of new team members, contributing to the development of a skilled and competent workforce.
    • Uphold ethical and legal standards of nursing practice and maintain confidentiality of client information.

    Qualifications:

    • Graduation from an accredited program for Licensed Practical Nurses (LPNs).
    • Current LPN license in the state of Pennsylvania.
    • Minimum of 2 years of clinical experience as an LPN, preferably in home healthcare or a related field.
    • Strong leadership and supervisory skills, with a demonstrated ability to motivate and inspire team members.
    • Excellent assessment, critical thinking, and problem-solving abilities.
    • Effective communication and interpersonal skills.
    • Ability to work independently and as part of a multidisciplinary team.
    • Proficiency in electronic medical records (EMR) systems.
