The Priority Care Center (PCC) Wellness Coach is an integral part of the Priority Care team. Wellness Coaches partner with patients to manage chronic health conditions and support patients in meeting their goals. A Wellness Coach may work with patients to achieve goals related to the domains of wellness such as, environmental, emotional, intellectual, physical, occupational, spiritual, social, and financial. Additionally, Coaches have training to support patients to lose weight, improve strength, reduce fall risk, quit smoking, manage stress and anxiety, improve sleep, and assist in client care coordination/management. They will work directly with patients, their providers, and the Priority Care team to provide support, guidance, education, and assistance to navigate health goals and the healthcare system. In addition, they will provide clerical and customer support and promote active and effective member participation.
Responsibilities:
- Wellness Coach:

Coaches provide face-to-face, telephonic, and written coaching, consultation to members managing various chronic health conditions with the clinical support and collaboration with the patient’s Primary Care Provider (PCP) and The Priority Care team.

Communicate with PCC members during daily and weekly huddles and refer to outside services when needed.

Promote active and effective member participation in care through an interactive process of assessment, member education, and collaborative problem solving.

Facilitate lifestyle change through a combination of strategies to enhance awareness, change behavior, and create environments that support good health practices.

Build rapport and establish a professional relationship with PCC patients.

Develop customized wellness plan for participants that includes SMART goals and a Wellness Vision

Efficiently documents results of member assessments, action plans, interventions, and outcomes into eCW (electronic medical record)

Participate in any community-based projects for prevention, including onsite classes.

Employee and Community Wellness:

Provide educational support to the School Based Wellness Program by participating in classroom educational activities and presentations.
- Assist program manager to support onsite health initiatives for employees.

Monthly wellness plans/board; daily activities; updates in weekly huddle.
- Promote environment that supports good health practices for all staff of the IPA.
- Team Support:
- Work as a member of the PCC team to ensure coherency amongst all of the team.
- Coordinates care with other members of the health care team/community for optimal patient outcomes.
- Attend department and other meetings as needed.
Qualifications:
- Bachelor’s degree or higher in health-related field

ACSM certified Health Fitness Specialist or related certification
- CPR certified.
- Motivational Interviewing Training preferred.
- Certified Wellness Coach from accredited coaching program preferred.
- Excellent communication skills for a diverse clientele of patients and clinicians

Detail oriented, well organized, and efficient.

Strong computer skills and proficiency in Microsoft Office Suite, including Word, Excel and Teams.
- Knowledge of specific disease and lifestyle related topics such as smoking cessation, weight management, nutrition, stress reduction, and chronic conditions.

CA drivers license and reliable transportation. This position will be required to travel to satellite sites within the county.
Preferred but not required experience, any of the following:

Yoga Instructor
- Three plus years of healthcare experience in Humboldt
- Addiction Management training
- Nutrition/cooking class
- Leading groups
