    SHIFT: 11AM-7PM

    Job Summary
    Provides pharmaceutical care to all areas of Ambulatory Care Services, including; Adult Emergency
    Room, Pediatric Emergency Room, Screening Clinics, Immediate Care Centers, Health Center Clinics,
    and Inpatient Discharge Services, depending on service level provisions. Outpatient front-line
    pharmacist assigned to the Ambulatory pharmacy setting. Supervises the performance of pharmacy
    students and pharmacy technicians in the Ambulatory Pharmacy setting.


    Typical Duties
    * Enters, checks, and fills prescriptions from all areas of Ambulatory Care Services using
    available automated systems
    * Checks drug orders for appropriateness and correctness; inspects drugs and pharmaceuticals
    in the clinics, on the pharmacy stations of the Ambulatory Care areas for their proper storage,
    adequacy and product integrity
    * Supervises pharmacy students and pharmacy technicians in their job function and job activity
    * Counsels patients on their medication usage
    * Evaluates and provides drug product information to the patient and to other health care
    professional
    * Advises physicians and nurses of proper selection of drugs and their possible effects and
    remains current with the appropriate state Laws and Regulations
    * Prepares stock requisitions; supervises, receives, inspects, and accounts for drugs and their
    proper storage; prepares and maintains the appropriate records that are required by Federal
    and State Laws and CCHHS Policies
    * Participates in Adverse Drug Reaction (ADR), Drug Utilization Evaluation (DUE) monitoring,
    hospital and departmental Quality assurance programs
    * Attends seminars, professional meetings and participates in educational programs
    * Participates in the filling and inventory record keeping of controlled substances

    Minimum Qualifications
    * Must be licensed as a pharmacist in good standing in the State of Illinois (MUST PROVIDE PROOF OF LICENSURE AT TIME OF INTERVIEW).
    * Graduation from a college of pharmacy accredited by the American Council on Pharmaceutical
    Education or equivalent (MUST PROVIDE OFFICIAL TRANSCRIPT AT TIME OF INTERVIEW).
    * Previous experience in a hospital or retail pharmacy

    Preferred Qualifications
    * A Doctor of Pharmacy (Pharm. D) (MUST PROVIDE OFFICIAL TRANSCRIPT AT TIME OF INTERVIEW)
    * Bilingual skills in English and Spanish
    * Previous experience with computerized physician order entry (CPOE) including basic
    knowledge and maintenance, PC-based word processing and Pyxis applications

    Knowledge, Skills, Abilities and Other Characteristics
    * Knowledge of hospital and pharmacy policies, procedures, and techniques, Federal and State
    Laws, and pharmaceutical entities available in the U.S. market and their proper nomenclatures
    * Knowledge of Pharmacokinetics, drug and poison information system, Drug Utilization
    Evaluation (DUE), and Adverse Drug Reaction (ADR) reporting systems
    * Dexterity to manipulate drugs and containers and labeling
    * Skill to compound and package medicines
    * Skill to use computer keyboard for prescriptions entry and to use typewriter, typing twenty-five
    (25) word per minute accurately
    * Ability and skill to communicate with a wide variety of individuals or groups
    * Ability and skill to maintain accurate records, perform within reasonably established standards
    of accuracy within the department
    * Ability to withstand the computer typing, prolonged periods of staring at the screen, and
    troubleshooting hardware
    * Physical dexterity and mental alertness
    * Ability to work under tension and at times under crowded conditions
    * Ability to walk and stand prolonged periods
    * Ability to maintain composure when dealing with patients who may be irritable and in discomfort
    * Ability to lift heavy objects; eyesight must be correctable to provide visual acuity of reading
    accurately
    * Ability to work various shifts
    * Willingness and ability to work other stations, and perform additional work assigned by
    management


    Physical and Environmental Demands
    This position is functioning within a healthcare environment. The incumbent is responsible for
    adherence to all hospital and department specific safety requirements. This includes but is not limited
    to the following policies and procedures: complying with Personal Protective Equipment
    requirements, hand washing and sanitizing practices, complying with department specific engineering
    and work practice controls and any other work area safety precautions as specified by hospital wide
    policy and departmental procedures.
