The Program Coordinator supports those in our care to reach their highest potential by providing them with assistance to achieve their goals and their daily activities. The Program Coordinator oversees the community-based programs providing a person center practice and evidence-based community setting to where individuals we serve can be independent. The Program Coordinator will be responsible for executing the growth action plan, also act as the liaison between the Support Coordinating Organizations and the provider to ensure that all individuals receive the necessary supports and services they require and desire while apart of the agency’s program.

RESPONSIBILITIES

• You must be familiar with Charting the Life Course, In-Home and Community Supports, Community Participation Supports, and Companion along with their codes (6100 and 6400).
• Meet weekly expansion growth quota by way of referrals that select our organization as the provider by way of Meet & Greets, ensure admission packets by both the individual and family are signed (upon entry to our program and annually
• Contact supports coordinator for follow-up questions regarding the ISP, report content discrepancy to the SC, as applicable, and plan team members coordinate Meet & greets ensure confirmation of IHCS.
• Participate in annual update and ISP revision, attending the ISP meetings, by providing last quarter progress notes and the next quarter, recommend vision to a service or outcome in the ISP, based on progress and/or lack of progress; therefore, you will supervise, monitor, and evaluate services provided to the individual.
• Completing, signing, and dating the monthly and quarterly documentation of an individual’s participation and progress toward outcomes. Complete the workflows in its entirety to ensure all steps are taken.
• Complete daily, monthly, quarterly, and annual status reports to include providing updates on your referrals that are in need of staffing.
• When a position is vacant and/or during down time, you make be asked to support other team projects such as recruiting and scheduling.
• Reporting a change related to the individual’s needs to the SC, as applicable, and plan team members.
• Coordinating the services provided to an individual by completing objectives. Program Planning & Compliance
• Facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, terminating, training, scheduling, conduct performance evaluations and oversight.
• Provide support to individuals receiving services including (you must be familiar with the ISP Manual and understand units, IHCS, CPS 100% in the community and Companion and their codes.
• You must also have the ability to convert units and hours for budgeting and monitor and track the completion of daily service notes.

EDUCATION/EXPERIENCE:

• Bachelor’s Degree in a human service field and 1-year related experience; OR
• Associate degree/60 credit hours in a human service field and 2 years related experience; OR
• High school diploma/GED equivalent and 4 years of combined human service, financial record keeping and maintenance experience.

LICENSE/CERTIFICATION:

• Driver's License (required)

Job Type: Part-time

Pay: $21.00 - $21.75 per hour
