Cooperative Program Coordinator coordinates cooperative education programs for the university/college. Treatment Coordinator

    Master’s Degree from an accredited college or university in a human services field with at least one (1) year of experience working with the population to be served preferred. Minimum of Bachelor’s Degree from an accredited college or university in a human services field with at least three (3) years of experience working with the population to be served. The Human Services field may include, but is not limited to, the following: psychology, social work, early childhood education, child development, or a related field, such as criminal justice, rehabilitative counseling, or elementary or secondary education. Must demonstrate sincere commitment and ability to work with and advocate for clients, their families, and foster families as well as the ability and willingness to work in partnership and cooperation with referring agencies, the community, and other agency stakeholders. Must be willing and able to do evening and weekend work, in and out of town travel, and overnight travel as needed and required.

    1. Report to the appropriate Director or his/her designee.

    2. Manage assigned caseload and provide support to clients and foster parents. This includes but is not limited to: the completion of a variety of assessment and information gathering on clients; the completion and documentation of home visits according to organization requirements; coordination of needed services for clients; monitoring of client placements and foster parent activities; and interfacing with referring agencies and other service providers of mental health, medical, educational, psychological, vocational, social services and other services.

    3. Work to prevent placement disruptions by being proactive and providing effective interventions with both clients and foster parents. Advocate with referring agency for needed services to help maintain placement stability.

    4. Ensure appropriate individual and group training of foster parents to build skills and meet organization requirements and as required by contract, Medicaid, COA and other funding sources.

    5. Provide RBHS interventions and complete corresponding documentation as well as other mandatory documentation according to organizational requirements and as required by contract, Medicaid, COA and other funding sources.

    6. Work to meet established client outcomes:

    • 99.68% of children/youth will not have any substantiated abuse of neglect while in care.
    • 65% to 75% of children/youth will be discharged to a less restrictive or permanent family setting within 9 to 12 months of admission.
    • 80% to 90% of children/youth will visit regularly with families and siblings in other placement settings in accordance with their care plans and, if in the best interest of all.
    • 75% to 85% of children/youth in a placement will remain stable.

    7. Maintain a positive relationship with referring agencies. Maintain regular contact with referring agency caseworkers for case planning and to encourage referrals to the SCYAP foster care program.

    8. Assist with the marketing of the SCYAP foster care program and actively be a part of the continued growth of the foster care program. Assist with recruitment and licensing of new foster homes. Assist with ongoing licensing of foster parents/homes as needed.

    9. Assist in family reunification efforts with the client’s primary family or other permanency plan. Advocate for siblings to be placed together whenever possible. Assist with maintaining sibling connections through coordination and/or facilitation of sibling visits when siblings are not placed together.

    10. Understand and follow SCYAP policies and procedures.

    11. Participate in and follow SCYAP procedures for after-hours and weekend emergency coverage.

    12. Participate in the quality assurance and improvement activities of the organization.

    13. Perform other duties as needed and assigned.
