Position Summary:
The person in this position will be responsible for planning and coordinating the clinical Substance Abuse/Mental health services for consumers in residential treatment. Conducts assessments for consumers and provides individual and family therapy. Provides direct supervision to staff assigned to program.
Essential Job Functions:
Oversee and coordinate clinical activities for residential programs.
Oversee and coordinate treatment services for consumers and their families with substance abuse and/or co-occurring disorders.
Responsible for reviewing case notes and weeklies on all active cases and makes comments and notes on to be discussed in the weekly Case Review Supervision meeting.
Meets individually with each staff, weekly, for a 60 90-minute supervision session.
Discusses progress or lack thereof, on each case and plan for next weeks sessions with Family Services Therapist.
Supervises staff meetings and weekly case conference, presentation and consultation of challenging cases, sharing of effective strategies.
Responsible for reviewing consumer assessments.
Responsible for reviewing Interpretive Summaries.
Responsible for reviewing Wellness and Recovery Plans monthly, and as necessary.
Document all clinical services and activities in accordance with licensing, Medicaid, and Joint Commission standards and agency policy and procedure.
Responsible for ensuring maintenance of clinical records in accord with licensing, Medicaid, and Joint Commission standards and agency policy and procedure.
Ensure appropriate collection and submission of clinical and evaluation data for the residential and outpatient programs (e.g. SISAR, ASAM, etc.)
Participate in case conference.
Represent the agency, as requested, at meetings of funding agencies, governmental authorities, seminars, workshops, etc.
Participate in the discharge planning process for the consumer and family.
Responsible for reviewing Discharge Summaries.
Determine eligibility of consumers for Medicaid or other third-party payment.
Provides referral and linkage for ancillary services for the consumer and family, as needed
Responsible for inputting and reviewing incident reports.
