POSITION SUMMARY: The Supervisor Digital Health will work with OSF OnCall clinical and nonclinical operations to increase the ability of those we serve to access health care and improve the health of vulnerable residents, as they care for themselves in the home and community setting. They will be responsible for relationship management with surrounding community based organizations. The Supervisor is responsible for supervising their direct reports and handling the scheduling of the team. They will be responsible for the day-to-day operations and activities of the Digital Health Workers (DHWs) and Digital Health Navigators (DHNs) in their respective hub.
REQUIRED QUALIFICATIONS: Education: Associates Licenses/Certifications: Community Health Worker certification within 1 year of hire into the position Experience: 1 year of experience in a service-related position Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented.
PREFERRED QUALIFICATIONS: Education: Bachelor's degree Experience: 2 years of health care operations experience Experience with social work or community health work Experience with change management and conflict management skills Licensure/Certification: Community Health Worker certification Other skills/knowledge: Proven ability to maintain professionalism with physicians, clinicians, and other professionals as well as community organizations Proficient in communicating in English, and by telephone, in person, and digitally Demonstrated ability to work independently
