Community driven care. Must be comfortable going into patient's homes for equipment education for RPM programs. Must be comfortable interacting with community partners. Must be comfortable with potential weekend work (2 shifts in a 7 week rotation is currently what we have).
POSITION SUMMARY: The Digital Health Worker (DHW) is a trusted member of the community who works in the community, with specific target populations, to promote healthy behaviors, build stronger relationships between the community and health and social services, and increase accessibility to those services. The DHW will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes. The DHW will visit patients virtually, checking on the patient's well-being and addressing social determinants that may hinder the patient from effectively caring for themselves. The DHW is locally situated to digitally enable our public health outreach to the communities we serve. The DHW will be responsible for deliveries to the patient's home to assist the patient in caring for themselves as well as enabling the patient to remain at home.
REQUIRED QUALIFICATIONS: Education: High School/GED Other Licenses/Certifications: BLS certification within 6 months of hire. Current driver’s license with own reliable transportation Community Health Worker (CHW) certification required or obtained within 6 months of employment. Experience: 6 months of experience in a service-related position Other skills/knowledge: Excellent interpersonal and communication skills.
PREFERRED QUALIFICATIONS: Experience: Multilingual Knowledge healthcare systems and outreach/networking skills with local agencies/community based organizations Other requirements/information: Demonstrated ability to communicate effectively by telephone; in person and digital (email, messaging, video chat) Proven ability to follow simple directions
