The Continuing Care Coordinator (CCC) assists residents/clients at Saint John Vianney Center (SJVC) with ongoing case management while patients are in treatment, with discharge planning and aftercare. The CCC works closely with referral sources (e.g., Dioceses, Vicars, and Superiors) to build and foster long-term working relationships. This position conducts on-site meetings for each resident on their caseload. The ideal candidate offers a well-rounded perspective with comprehensive patient care. The CCC should be a role model for excellence in customer service with residents and their superiors through communication, collaboration and coordination of treatment and aftercare services.
