Public Health Case Coordinator

    The Public Health Department is currently accepting applications to fill five (5) Full-Time, Public Health Case Coordinator to fill our two (2) Bilingual Spanish, Department Service Navigator and three (3) Enhanced Care Coordinator vacancies within the Primary Care and Family Health (PCFH) program. These roles can report from any location, and open to a hybrid work schedule. The County classification title is Health Services Aide.

    POSITION: 
    Department Service Navigators will work as a part of a collaborative inter-Departmental team to meet community Public Health needs. Under general supervision will support overall clinic operations including assisting clients with intake survey; providing translation services (both written and oral); assisting with client flow; and performing other duties as assigned to meet organizational needs. Will work as a part of a collaborative team to meet community Public Health needs.
     
    Enhanced Care Coordinators supports program growth by making initial contact in engaging with potential patients, providing information about Enhanced Care Management services, and enrolling patients. Under the direction of the ECM Registered Nurse Supervisor, the Enhanced Care Coordinators will determine eligibility, complete enrollment assessments and work with program partners to oversee the eligible patient population for the ECM Program. The Enhanced Care Coordinator carries a caseload of eligible ECM participants and is responsible for conducting outreach, enrollment, referrals, coordinating health care services, providing education, and documenting activities while supporting member access to care and resources. The Enhanced Care Coordinator convenes and facilitates interdisciplinary team meetings with those providing care or other support services. The Enhanced Care Coordinator works alongside their clients, establishing strong communication and collaboration in their plan of care, as well as works with caregivers/family members, other support persons and providers, to promote wellness, independence, resilience, and empowerment.
     
    DISTINGUISHING CHARACTERISTICS: This classification is distinguished from Health Services Aide, Senior in that the emphasis of Health Services Aide is on clinic assignments while the focus of Health Services Aide, Senior is on outreach that is accomplished by conducting home visits.
     
    The IDEAL CANDIDATE will possess: 
     
    1. Bilingual Proficiency: Fluent in both English and Spanish, with the ability to accurately translate complex circumstances and medical information.
    2. Exceptional Communication Skills: Demonstrated ability to effectively communicate with diverse populations, both internally within the team and externally with clients and partners.
    3. Strong Organizational Abilities: Proven capability to organize tasks, prioritize responsibilities, and manage multiple projects simultaneously in a dynamic work environment.
    4. Effective Time Management: Ability to efficiently allocate time and resources to meet deadlines and ensure timely completion of tasks while maintaining quality standards.
    5. Proficiency with Software Tools: Experience using Smartsheet and/or other spreadsheet software is preferred, with the ability to quickly learn and adapt to new technologies and platforms.

    1. Participates in outreach and engagement efforts to enroll clients in appropriate programs.
    2. Carries a caseload of clients and provide case management, including but not limited to assisting clients with care coordination needs, referral support, community resource connection, health care navigation, and tracking all referrals.
    3. Assists clients in navigating the healthcare system, helping them to successfully participate in their medical and/or behavioral health care by identifying and overcoming barriers. 
    4. Interviews clients to collect medical history data and social determinants of health for review by healthcare professionals and care team members.
    5. Completes duties in the office, through field-based outreach, and via meetings with service providers. Locations for service delivery may include the office, clinics, homeless shelters, homes, and on the street.
    6. Engages with clients, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as Motivational Interviewing) to promote collaboration between the client and their care/service team, as well as to increase the client's sense of control over their whole health.
    7. Ensures clients receive information that is culturally respectful, including using translation services when necessary. 
    8. Coordinates interdisciplinary team sessions with members from the clinical care teams and other service providers, such as housing.
    9. Offers ongoing support, outreach, and engagement to connect clients with Santa Barbara County departments.
    10. Collaborates with external partners to ensure comprehensive support for clients.
    11. Utilize a tailored approach to help clients navigate issues affecting their social determinants of health.
    12. Disseminates pertinent information through various communication channels.
    13. Cultivates and maintain critical partnerships with departments and external stakeholders to enhance support services.
    14. Design, plan, implementation, and evaluation of support services tailored to the needs of diverse community populations.
    15. Acts as a primary point of contact for clients, facilitating referrals to relevant departments.
    16. Coordinate participant tracking to assess the effectiveness of programming in supporting clients.
    17. May transport clients.
    18. Performs additional duties as assigned to support organizational goals.
    Any combination of training, education, and experience that would provide the required abilities.
     
    Desirable Qualification: Experience working in a health care or human services setting is highly desirable.
     
    Ability to: understand the needs and problems facing culturally, economically, or socially disadvantaged persons; establish and maintain good relations with persons from a wide range of social and ethnic groups; learn the range of departmental services for the treatment of health, nutrition, or other related problems; communicate effectively orally and by writing simple narrative reports; maintain confidentiality of client information; understand and follow oral and written instructions; work cooperatively as part of a health services delivery team.APPLICATION & SELECTION PROCESS: 
    1. Review applications and supplemental questionnaire to determine those applicants who meet the employment standards.  
    2. Supplemental Questionnaire Ranking. Responses to the required supplemental questionnaire will be evaluated and auto-scored. Candidates’ final score and rank on the employment list will be determined by their responses to the supplemental questionnaire. This process may be eliminated if there are fewer than 15 qualified candidates.  

    Candidates must receive a percentage score of at least 70 on the supplemental questionnaire ranking to be placed on an employment list. An adjustment may be made to raw scores based on factors listed in Civil Service Rule VI. Those candidates who are successful in the selection process will have their names placed on the employment list for a minimum of three months. At the time the employment list is established, all candidates will receive an email notice of their score on the exam(s), rank on the employment list, and exact duration of the employment list.    
     
    VETERAN PREFERENCE POINTS: Veteran’s preference credit is applicable for this recruitment (5 points for veterans, 10 points for disabled veterans). To be eligible for this credit, you must be applying for this position within five years from your most recent date of: (1) honorable discharge from active military service; or, (2) discharge from a military or veterans’ hospital where treatment and confinement were for a disability incurred during active military service; or, (3) completion of education or training funded by a Federal Educational Assistance Act.  No time limit exists for veterans with 30% or more disability.
