    PART TIME-(MAYWOOD, CA) - 20 hours/weekly

    We are seeking a Service Coordinator to provide service coordination and oversee case management for resident members in CSI Support & Development co-ops. The Service Coordinator's responsibility is to empower the resident member to age in place and enhance the ability to members to remain living independently in the co-op. This position requires demonstrated experience in coordinating community services for older adults. The successful candidate will have effective interpersonal skills as well as program planning, general office, and computer skills. Candidates with these skills along with being organized, flexible, knowledgeable and fluent in Spanish, will be successful supporting the resident members of our consumer cooperatives as they live, play and volunteer in our superior and affordable senior housing communities.

     

    Duties include but are not limited to:

     

    • Assist and educate members and families of services which may be necessary to maintain an independent lifestyle;
    • Assessing resident members needs for health, psychological and social services to identify needs;
    • Assist members in building informal support networks among themselves and with family members and friends in their community;
    • Educate resident members, their families and CSI staff on available community resources;
    • Act as a liaison between community agencies, service providers and resident members;
    • Facilitates meeting of needs when necessary while discouraging unhealthy dependence;
    • Work collaboratively with the members, council, co-op liaison and other committees of the co-op;
    • Establish relationships with local service providers who are currently providing or could provide services in the future;
    • Monitor the quality and effectiveness of service delivery;
    • Plan and implement dynamic educational and group programs; 
    • When requested, work with staff to mediate conflicts between resident members;
    • Maintain a directory of community services and make it available to resident members, their families and staff;
    • Maintain and update individual member files with member contact, providers and family/friends;
    • Pursues continuing education in services geared toward the elderly;
    • Reporting and distribution accurately to meet deadline;

     

     Requirements:

     

    • Bachelor's degree or higher in social work, psychology, gerontology, counseling or related specialty, 1 years of experience working with older adults in a community-based setting or equivalent combination of education and experience;
    • OR  an associate's degree with 3 years of experience in case management or service coordination with older adults in a community-based setting;  
    • Fluent in Spanish a plus;
    • Good oral and written skills;
    • Working knowledge of social work terminology and reporting processes;
    • Working knowledge of senior community and area resources;
    • Ability to work effectively with people in a multi-cultural environment;
    • Experience in health & wellness and grief counseling desired;
    • Proficiency in Microsoft Office Products;
    • Ability to work independently;

     

    Working Conditions:

     

    • Ability to work with challenging clients;
    • May have to enter apartments in various conditions regularly;

     

    Physical Requirements:

     

    • Must be able to lift/push up to 20 pounds;
    • Ability to sit for extended periods of time;
    • Frequent use of keyboard and phone;
