Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.     Job Description:

    • The Diabetes Educator is a registered nurse, who is committed to promoting wellness and growth toward self-reliance for staff, patients, and the community. The age of the population served includes pediatric, adolescent, adult and geriatric. Responsible for assessing diabetes education needs of individuals and the community and coordinating resources to meet these needs.
    • Collaborates with nursing staff in the daily identification of patients who need instruction and follow-up education.
    • Assists in organizing, planning and offering the community diabetes classes, programs or activities that will create health awareness, assist in lifestyle changes and teach prevention of unfavorable outcomes.

    Job Qualifications:

    • Graduate of an accredited nursing school required
    • Current Indiana license required
    • Certification as a Diabetes Educator within 2-3 years of assuming position, required
    • Previous experience in clinical nursing practice
    • Specialized knowledge and care of patients with diabetes

    FTE: .8 (64 hrs/wk) M-F Days
