Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.     General Summary
     

    Under general supervision, assesses, plans, implements and evaluates the multidisciplinary diabetes education program at the University of Maryland Rehabilitation & Orthopaedic Institute.  Plans and conducts continuing education programs in diabetes for allied professionals and the general public.  Functions as a resource for patients and healthcare professionals within and outside the Rehabilitation & Orthopaedic Institute.

    Principal Responsibilities and Tasks
     

    The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification.  These are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

    1. Provides oversight and coordination supporting a robust enterprise-wide diabetes management and education program.
    2. Collaborates with physician leadership, administration, professional and other team members throughout the organization, as well as community health and social services resources to sustain and grow the diabetes programs and population health department.
    3. Promotes referral development for inpatient diabetes education liaison.
    4. Plans, assists and implements the development and coordination of educational programs and seminars focusing on the management of diabetes, diabetes- related problems and diabetes education for healthcare professionals and support staff.
    5. Participates on grant initiatives as requested. Maintains and provides reports to Administrative personnel as required.
    6. Educates and counsels patients with diabetes and their families, develops holistic care plans and coordinates patient care across disciplines.
    7. Functions as a collaborative member of a multidisciplinary team.
    8. Assists in development of clinical practice protocols and standards for the unit based on evidence.
    9. Coordinates participation in research programs and studies as assigned.
    10. Participates in continuous quality improvement projects.
    11. Maintains a level of quality and safety by holding self and others accountable for meeting regulatory standards. Participates in departmental traces, mock surveys and mock and other safety efforts.  
    12. Serves as an active participant in unit based and hospital-wide governance activities including clinical practice, wellness and safety councils.

    16. Follows policies, procedures and lesson plans, as provided by the UM Center for Diabetes and Endocrinology, and assists with review and revision on an annual or as-needed basis.

    Patient Safety

    Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.

     

    1. Takes action to correct observed risks to patient safety
    2. Reports adverse events and near misses to appropriate management authority.
    3. Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.

    Qualifications

    Education and Experience

    1. Bachelor’s degree in Nutrition is required
    2. Certification as a Diabetic Educator (CDE) is required.  
    3. Minimum of four years as a diabetes educator is required.

    Knowledge, Skills and Abilities

     

    1. State of the art knowledge of utilization and population health tactics.
    2. Demonstrates expertise in health information functions, operations and medical staff relationships, and privacy/release of information. Able to interpret contents and relay information to others as needed.
    3. Demonstrates proficiency with knowledge of Microsoft applications.
    4. Demonstrates knowledge with diabetes self-management education and counseling techniques.
    5. Ability to develop market and instruct diabetes education.
    6. Ability to assess all diabetes programs and services according to objectives.
    7. The ability to ensure the confidentiality and rights of all client/patient, health system and departmental documents is required.
    8. Excellent verbal and written communication skills are required. The ability to communicate effectively both verbally and in written form with physicians, contractors, health system officials, other health system officials, other health system personnel and the public is essential.
