Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.     Overview

    We are looking for highly qualified and truly compassionate nurse practitioners and physician assistants for our location in Midland Commons, a brand-new center at 6901 Ray Wright Rd, Columbus, GA 31906. This position is ready to start as early as Summer 2024.

    Job Description

    • Evaluate, diagnose, and treat patients with diabetes, prediabetes, and other related comorbidities.
    • Conduct patient interviews and collect patient histories, psychosocial histories, health status, and medical/potential problems.
    • Order, schedule, and/or perform laboratory tests and diagnostic procedures such as blood tests and urinalyses. Collect and interpret diagnostic data to implement appropriate care patient care plans.
    • Provide behavioral health coaching and educate patients on health habits, self-monitoring, nutrition, physical activity and movement, sleep, stress, at-home medical care, medications, and other issues related to their medical or health problems.
    • Maintain and update current, accurate patient records including medical assessment, interventions, and evaluations.
    • Provide updates on patients’ health status and progress to patients, patients’ family members, and/or designated caregivers as appropriate.
    • Participate in community outreach and collaborate with local healthcare professionals for any specific patient medical needs, including pharmacy, primary care, cardiology, nephrology, neurology, optometry, and podiatry.
    • Stay abreast of and excel at utilizing the latest therapies, technologies, and best practices in diabetes, prediabetes, and other related comorbidities. Maintain expertise in clinical guidelines such as ADA, AACE, AHA, ACC, AND, and KDIGO.
    • Maintain technical expertise in digital solutions, including all administrator, provider, and patient-facing applications, pharmacotherapy decision support tools, externally integrated solutions, supported medical devices, electronic health records, scheduling tools, and newer AI-enabled solutions.
    • Collaborate with other team members to regularly evaluate, refine, and improve processes in the clinics, tools being used, and patient experience.
    • Assist with the screening and hiring of new team members as needed.

    Qualifications

    • Completion of an accredited nurse practitioner or physician assistant program.
    • Currently licensed as an advanced practice nurse in the state of Georgia by the Georgia Board of Nursing and certified as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC) or equivalent body, or Certified as a Physician's Assistant by the National Commission on Certification of Physician Assistants (NCCPA).
    • Licensed as appropriate in the state of Georgia.
    • Two (2) or more years of experience managing patients with diabetes and/or prediabetes in a clinical setting.
    • Exceptional leadership, organizational, communication, and interpersonal skills.
