Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.     GENERAL OVERVIEW:

    Assesses patient need for diabetes education. Provides individualized diabetes self-management education/training based on assessment. Plans and conducts continuing education programs for professional staff and the community.

    ESSENTIAL RESPONSIBILITIES:

    • Collects DSME/T program participant assessment data, using a recognized certification database/program in a collaborative and ongoing manner.
    • Develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.
    • Collaboratively develops an individualized follow-up plan with each program participant. Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.
    • Conducts a follow-up assessment upon completion of DSME/T program services, using outcome measures from the AADE7 Continuum of Outcomes Framework.
    • Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records.
    • Participates in the development of education curricula and materials.
    • Performs other duties as assigned or required.

    QUALIFICATIONS:

    Minimum

    • Bachelor’s Degree
    • Registered/Licensed Dietitian.
    • Certified Diabetes Care and Education Specialist (CECES)
    • Experience in counseling patients with diabetes and experience in community education.

    Preferred

    • Master’s Degree.
    • 1-3 years’ experience in Diabetes Education / Health Living Coaching.
    • Certified insulin pump trainer (CPT).
