Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. Summary:
The Inpatient Diabetes Educator is responsible for providing consultation, screening, assessment, reassessment, and education for patients with a variety of medical conditions and members of the interdisciplinary care team on the acute care units at Prairie Ridge Health (PRH). They initiate and complete inpatient consults including order set implementation, medication management and other appropriate orders and interventions using facility approved Standing Orders. Participates in developing and updating procedures, protocols and departmental policies. Facilitates compliance with The Joint Commission, diabetes program recognizing body, state and other regulatory agencies related to nutrition and/or diabetes care. Participates in quality initiatives.
