    Overall Objectives of Position
    The Diabetes Case Manager is responsible for diabetic case management activities and assistance to the medical providers and diabetes team in coordination with the goals of the diabetic program. This job description is not an all-inclusive list of the duties and responsibilities of this position. The Diabetes Case Manager is expected to perform all duties and responsibilities necessary to meet the goals and objectives of applicable programs.
    The Diabetes Case Manager is expected to live the Purpose and Values of the Poarch Band of Creek Indians and will go beyond the call of duty.

    Primary Responsibilities of the Diabetes Case Manager
    • Maintains diabetes patient registry and submits required reports to Indian Health Service (IHS) and United South and Eastern Tribes (USET).
    • Oversees case management of patients diagnosed with diabetes who are in need of care coordination.
    • Oversees Special Diabetes Program for Indians (SDPI) grant including renewal application and annual budget.
    • Adheres to SDPI grant requirements and directives.
    • Records, prepares and submits data to maintain American Diabetes Association (ADA) recognition for diabetes self-management and education program.
    • Plans and coordinates classes including but not limited to: diabetes prevention, self-management and support.
    • Actively participates in GPRA meetings and leads measures related to diabetes.
    • Responds to consults within three (3) business days.
    • Responsible for SDPI GSA vehicle – scheduling maintenance and upkeep.
    • Maintains inventory of SDPI program supplies and replenishes as needed.
    • Participates in community health events related to diabetes education and health promotion.
    • Participates in Quality Improvement Team meetings and provides pertinent data and reports.
    • Educates patients on application, troubleshooting, and care of continuous glucose monitoring (CGM) systems.
    • Develops and administers departmental policies and procedures as necessary.
    • Participates in Pharmacy and Therapeutics (P&T) Committee meetings and provides updates per current ADA standards of care regarding medications and treatment options.
    • Makes home visits when applicable to provide education or CGM training to patients.
    • Completes prior authorizations for medications used to treat diabetes or equipment used to monitor blood glucose.
    • Leads annual and as needed Diabetes Advisory Team meetings to ensure compliance with ADA recognition requirements.
    • Promotes diabetes prevention and awareness in the community through development and distribution of educational materials and by providing community classes.
    • Performs other duties as assigned by the appropriate person.
    Day-to-day Responsibilities
    • Assess educational needs of patients diagnosed with diabetes or at risk for diabetes and provide education and support tailored to the patient’s learning style and level of understanding.
    • Coordinate patient care with the providers and other members of the health care team to ensure that standards of care are met.
    • Acts as liaison between patients and providers, if changes are required in treatment and/or medication.
    Education/License/Certification and Experience Requirements
    • Bachelor’s Degree in Nursing required.
    • Possession of a certificate of registration to practice professional nursing as issued by the Alabama Board of Nursing.
    • Minimum of five (5) years of employment experience in diabetes patient care preferred.
    • Must maintain current professional licensure in Basic Life Support (BLS) or obtain licensure within ninety (90) days of date of hire.
    • Must be a Certified Diabetes Educator or must obtain certification as Certified Diabetes Educator within one (1) year from date of hire.
    • Must possess a Case Management Certification or must obtain certification within eighteen (18) months from date of hire.
    • Must possess certification of “Mandatory Reporter Training” or obtain certification during the first week of employment.
    Skills Required
    • Absolute confidentiality, adhering to HIPAA guidelines and the Privacy Act.
    • Ability to work in a high-performance, fast-paced, high-pressure environment.
    • Must be able to work in a team environment and independently.
    • Must be knowledgeable of Type 1, Type 2, and gestational diabetes.
    • Must have a working knowledge of computers and electronic health records, with the ability to work in a Windows environment with various software programs such as Microsoft Office software and etc.
    • Must be able to read, analyze, and interpret business periodicals, professional journals, technical procedures, and/or government regulations. Attention to detail and organizational skills a must with the ability to carry our comprehensive research work.
    • Must be people oriented and relate well to people from diverse backgrounds.
    • Must be comfortable with public speaking and able to confidently address and manage an audience.
    • Must be able to write reports and effectively communicate with health care providers, patients, Tribal and Indian Health Services staff, and the general public.
    Additional Requirements
    • Must possess a valid state driver’s license and insurable driving record according to the Tribal insurance guidelines.
    • Must successfully pass the required criminal and character background check.
    • Ability to travel and participate in required training, leadership development, and other events.
    • Ability to adequately and successfully perform all duties and responsibilities of this position.
