Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. Summary
Maine Medical Partners has an opening for a Diabetes Educator-Registered Nurse. The candidate joining our practice will support patients living with diabetes through 1-1 patient visits and on-going classes. Our programs are recognized by the American Diabetes Association and emphasize a patient-coaching and team-based approach. Topics include nutrition counseling, diabetes self-management, and training on innovative technology: building alternative class modalities delivering methods to include on demand and web-based technologies.
An RN license and 2 years of nursing experience are required for this position. If you are not currently credentialed as a Diabetes Educator, we will work with you to obtain the credential within your first two years.
