Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. The Diabetes Program Assistant position is responsible for assisting the Diabetes Program Manager (DPM) and Diabetes Department employees with DPP and Diabetes classes, retinal screens and other administrative duties as needed. Must be able to travel to both Grass Valley and Auburn locations.
