Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.     Your Role With Us:

    The Health Educator provides individualized and group education and services to patients, families and community members on selected health topics. Interacts positively with individuals, families, and large community groups in the course of delivering services. Applies educational principles and process to assess, plan, deliver and evaluate education and services. Participates in planning, staffing, and evaluating outcomes of health promotion and health education classes and events.

    Background/Expertise:

    • Bachelor’s Degree in health-education, community or public health, nursing, nutrition or health and wellness related field
    • One (1) year experience providing health education/instruction required.
    • Relevant specialty certification(s) and membership in related professional organization(s) is desired.
    • Bilingual English/Spanish preferred.
