Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.     Job Summary and Qualifications

    As a Diabetes Educator at our facility you will have the opportunity to be both a nutrition professional and a caregiver, leading a hard-working and compassionate team that is so vital for the day-to-day functions of our hospital. In this role:

    • You will provide education for diabetes and other disease management for inpatient hospital staff, residents, and health care providers.
    • You will be responsible for the assessment of patient nutritional status and planning, coordination, implementation, and evaluation of nutritional care plans.
    • You will educate patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity, and glucose testing.
    • You will conduct outpatient nutrition counseling and facilitate individual counseling sessions and group classes.
    • You will teach and coordinate community classes.
    • You will support Quality Management and Organizational Development departments in the improvement of inpatient diabetes care.
    • You will communicate with patients and families, medical staff, nursing staff, and other health care professionals to assure patient care needs are met.

    What qualifications you will need:

    • Must be a Certified Diabetic Educator (CDE) or have current Utah licensure as a Registered Dietitian or Registered Nurse.
    • Must maintain appropriate continuing education credits within a 5-year period as specified by the commission on Dietetic Registration (CDR).
    • Bachelor’s or Master’s degree from an accredited college or university is required.
    • Prefer a minimum of 2-4 years’ experience in diabetes education or outpatient counseling in an acute care facility.
    • Experience with insulin pumps and CGM a plus.
