Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. Job Description:
This position provides specialized education for patients, families, and members of the community. The RN Patient Family Educator position conducts training from approved curriculum resources. This position may conduct needs assessment and individualize educational activities as appropriate for participants. This position participates in curriculum updates and revisions based on current research and policy revisions.Position will be providing Outpatient Diabetes Education.
Hours: Daytime M-Th. 8 hrs a week - either all in 1 day or split into 2 shorter days.
Previous diabetes education experience and/or CDCES certification preferred
