Position Description:
We are looking for a CBIT for Kids Blended Case Manager to join our Team!
Nulton Diagnostic & Treatment Center is accepting applications for a CBIT for Kids Blended Case Manager. The successful candidate will Provide Blended Case Management Services in accordance with Federal, State and Agency requirements, and will complete and satisfy all quality and performance markers set forth by Nulton Diagnostic and Treatment Center in relation to the fidelity of the program.
The principal responsibilities of the Blended Case Manager will be Linking the consumer with Services, Monitoring of Service Delivery, Gaining Access to Services, Assessment and Service Planning, Problem Resolution and Informal Support Network Building.
The Blended Case Manager must be available to provide intakes services to a client within 24 hours of our agency receiving the referral and be available to provide On-Call services rotations.
Education, Experience, Skills, Training:
The blended case management staff person shall meet one of the following criteria:
A. Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,
B. Registered nurse; or
C. A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years’ experience in direct contact with mental health consumers; or
D. A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.
