Blended Case Manager
Position Description:
We are looking for a Blended Case Manager to join our Team! Please note this is not a Nursing Case Management job, but an entry level Behavioral Health Case Management position.
Nulton Diagnostic & Treatment Center is accepting applications for a Blended Case Manager in Blair County. The successful candidate will have knowledge of mental health services for adults and children/adolescents. This full-time position will assist individuals in accessing, monitoring, linking, and coordinating mental health treatment while providing needed advocacy and support.
Education, Experience, Skills, Training:
Minimum Education:
A. Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,
B. Registered nurse; or
C. High school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two year's experience in direct contact with mental health consumers; or
D. High school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.
The successful applicant must have a valid Driver's License and be able to obtain Act 33, Act 34 and FBI clearances.
