Dietitian - Home Care conducts assessments of patient/caretaker nutritional needs to implement nutritional care plan complying with the medical diagnosis needs. Plans menus, inspects food to ensure portions adhere to set restrictions and dietary standards. Being a Dietitian - Home Care requires a bachelor's degree. Requires state license to practice. Additionally, Dietitian - Home Care requires Registered Dietitian (RD). Typically reports to a supervisor or manager. The Dietitian - Home Care occasionally directed in several aspects of the work.     Caregiver responsibilities:

    • Daily personal care such as bathing, grocery shopping, oral care, cooking meals, etc.
    • Assist and provide help with the daily living activities per the Plan of Care
    • Track your patient’s condition and/or report to the agency if there is something unusual
    • Assist patient with daily meals 
    • Basic housekeeping functions
    • Accompany patient to doctor’s appointments
    • Assist with essential errands
    • Communicate with Summit regarding any issue that your patient might have
    • Follow agency’s policies

    Caregiver requirements:

    • New York Home Health Aide Certificate or Personal Care Assistant Certificate
    • Government issued ID
    • Social Security Card
    • Work permit, if needed
    • 2 Reference letters
    • Recent physical
    • Quantiferon 
    • Rubella and Rubeola lab work
    • Chain of Custody Drug Screen, no older than 6 months
    • Flu vaccine
