Additionally, Dietitian - Home Care requires Registered Dietitian (RD). Typically reports to a supervisor or manager. The Dietitian - Home Care occasionally directed in several aspects of the work. Minimum Qualifications:
- At least 1 year of experience in a hospital, nursing care facility, or similar medical setting
- Bachelor of Science degree from an accredited institution
- RD or CDN preferred, but not required
Job Responsibilities:
- Utilize EMR experience to conduct clinical dietary assessments, develop care plans, and complete necessary documentation such as MDS, readmission/new admission assessments, MNA, and baseline care plans
- Perform nutritional assessments for residents, including admission, readmission, quarterly, and annual evaluations
- Develop individualized care plans tailored to residents' nutritional needs
- Actively participate in food service department meetings and comprehensive care plan meetings
- Provide supervision and support to kitchen staff and tray line operations when necessary
- Ensure ongoing compliance with regulatory agency requirements
