Dietitian - Home Care conducts assessments of patient/caretaker nutritional needs to implement nutritional care plan complying with the medical diagnosis needs. Plans menus, inspects food to ensure portions adhere to set restrictions and dietary standards. Being a Dietitian - Home Care requires a bachelor's degree. Requires state license to practice.     Representative Functions:

    • 1. Conducts initial comprehensive nutritional assessment and comprehensive plan of care within first 13 treatments or 30 days, whichever is first, for review by the interdisciplinary team. Completion of assessment and plan of care will be completed annually unless patient meets unstable criteria.
    • 2. Reviews patient's lab values monthly for trends and correlations. Discusses lab values with the physician and reviews results with the patients on a monthly basis.
    • 3. Establish nutritional goals for patients and continue to monitor patient progress and readjusts goals that are not met.
    • 4. Complete and document ongoing assessment, evaluations and monthly notes, along with documenting and monitoring ongoing nutritional issues and goals.
    • 5. Assesses new patients utilizing dietary intake history, patterns and information for planning as well as ongoing care for all aspects of ESRD patient’s nutritional needs.
    • 6. Records, reaction to and comprehension of diet instructions and notes on the nutritional progress of the patient information in the patient’s permanent record
    • 7. Participates in health team rounds assisting the physician in establishing the proper diet order according to the patient's specific co-morbidities and requirements.
    • 8. Prepares and participates in QAPI facility team meetings.
    • 9. Acts as a consultant and resource person to physicians, nurses and other members of the health care team in the area of renal diets as well as in all areas of normal and therapeutic nutrition.
    • 10. Meets with dialysis patients to ascertain and ensure proper dietary requirements/restrictions are being met recommend appropriate meal patterns and/or formulas and supplements.
    • 11. Consults with other members of the interdisciplinary team and other medical staff regarding the nutritional care of patients and their progress.
    • 12. Counsels the patient and his/her family on the rationale of the diet and/or meal plan that has been developed.
    • 13. Conducts group classes with the patients, families and nursing home staff to reinforce their understanding of renal diets.
    • 14. Conducts in-service education for other members of the interdisciplinary team.
    • 15. Compiles or develops education material and uses them as aids in nutritional education.
    • 16. Participates in continuing education opportunities in order to update knowledge and keep abreast of changes in diet therapy in ESRD and other related fields.
    • 17. Interprets, evaluates and utilizes current research related to renal nutritional care.
    • 18. May perform other duties as deemed necessary by the Nurse Manager.
    • 19. Comply with policy and procedures and in accordance with state and federal regulation for patient care and services.
