Plans menus, inspects food to ensure portions adhere to set restrictions and dietary standards. Being a Dietitian - Home Care requires a bachelor's degree. Requires state license to practice. Additionally, Dietitian - Home Care requires Registered Dietitian (RD). Typically reports to a supervisor or manager. The Dietitian - Home Care occasionally directed in several aspects of the work. You will work on a full-time basis to oversee and provide the best continuity of patient care by acting as a liaison among hospitals, doctors, homes and home care agencies.
Essential Functions: This job requires an employee to stand, talk and hear. There is a frequent standing and walking, and occasionally to sit, bend, kneel, stoop or crouch. This position requires frequent use of hands and fingers and may require reaching in front or above shoulders, feeling or grasping.
Establish and maintain good relationships with hospitals, nursing services, social services, therapy officesand various other medical and non-medical support services Communicate personally with patient care managers to ensure standards of care are being met and thatpatient needs are being identified and met on an ongoing basis Recognize short-term and long-term topics that need addressing, and work on finding solutions Chat with hospitals, doctors, prospective clients and their families about the home care services theagency provides Gather and prepare information in advance of patient transfers; track data related to such transfers andcompile information into reports such as statistical reports Keep communication smooth between hospitals and doctors and home care agencies by continuallyreporting patient progress and therapies received Develop and assess policies for home care, including provisions for meeting federal, state and localregulations as well as professional and licensing standards
Pennsylvania RN, LPN, Therapist license, preferred. Some previous experience in some sort of medical liaising job (private case management, for example) Experience in home health, post-acute care is preferred. Excellent oral and written communications Bilingual skills (particularly in Spanish) beneficial but not required
