Following a Nutrition Care Process – Assesses, plans, coordinates, monitors, and evaluates nutritional care with regard to diagnosis, individual needs, current nutritional interventions, and in accordance to established hospital policies. The Clinical Dietitian’s primary focus is the in-patient setting with some outpatient functions, as needed.
Essential Duties & Responsibilities
- Checks that the diet is appropriate to the diagnosis for entire length of stay and any changes in status/diagnosis

Diet history screenings done by nursing are reviewed by the dietitian for accuracy by conducting a pre-assessment screen in a prompt and accurate fashion according to information available in the EMR. This is the basis on whether or not a full nutritional assessment is needed according to a pre-determined Nutrition Risk Factor, per protocol.

Performs and documents accurate and timely nutritional assessments/ reassessments according to Department protocols using the Nutrition Care Process and correlates with other healthcare plans.

Performs assessments and reassessments using appropriate Standardized Nutrition Language and avoiding dangerous abbreviations. The assessment includes, as available, a Food/Nutrition-Related History, Biochemical Data, Medical Tests, and Procedures, Nutrition-Focused Physcial Findings, and Client History. A current Nutrition Risk level is also assigned.
- An accurate and appropriate Nutrition Diagnosis is made and documented which includes PES (Problem, Etiology, Signs/Symptoms) statements with the etiology relating to the problem/s that can be treated.

Documents and arranges for appropriate Nutrition Interventions and Goals that correspond to the most significant nutrition diagnosis that can be impacted while the patient is in the hospital. Ensures the plan is carried out and communicated to other team members. Interventions may include recommendations, arrangements, and monitoring plans (Goals) as they relate to the Food and/or Nutrient Deliver, Nutrition Education, Nutrition Counseling, and Coordination of Nutrition Care.
- Monitors, evaluates, and documents outcomes of patient nutritional care appropriately by collecting and reviewing data pertaining to interventions and goals selected for the patient (i.e/ Appetite Checks, Calorie Counts, Parenteral/Enteral Intake, Wt. history, Lab Changes, Skin Changes.)

Arranges, documents, and provides diet education and counseling for individuals and families

Education and counseling are done in a timely manner considering current nutritional principles, dietary plans, food selection, and economics by adapting plans to the individual's lifestyle and learning needs.

Utilizes appropriate tools in the provision of nutritional care, by using a current diet manual, nutrition references and nutrition software.
- Assists and supports the Director of Nutritional Services, as needed for other duties requested
- Examples of other duties but not limited to: out patient scheduling, counseling, patient menu planning, in-service education, Cardiac Rehab, Diabetes Self Management Educationand Support Services, and other program/departmental needs.

Routinely plans and organizes resources and time to get the job done.

Participates in continuing education opportunities in a manner consistent with the requirements to remain a registered and licensed dietitian.

Maintains accurate and timely records that are readily available, including registration, licensure, in-services, education, Care Learning, and competency.

Helps to maintain safety, sanitation, and infections control standards (i.e./ hand hygiene monitoring)

Monthly, records Performance Improvement data and assists in Quality Improvement plans, and Departmental Quality Control Stats in a timely manner.

Evidences the knowledge and ability to assess and interpret data about the patient’s growth and development in order to identify each patients needs and provide the appropriate care. Includes age related care where appropriate:
- Infant

Pediatric

Adolescence

Geriatric
- No one but a licensed and registered dietitian can legally perform nutritional assessments. The practice has specific guidelines that are unique to the profession, such as the International Dietetics and Nutrition Terminology which spells out a Standardized Language for the Nutrition Care Process which is very prescriptive

Complies with federal, state, and local legal requirements by studying existing and new legislation; anticipating legislation; enforcing adherence to requirements; advising management on needed actions; ensures compliance with Joint Commission and other external and internal regulatory agencies.

Maintains the integrity of confidential patient information in accordance with HIPAA and hospital policy, including accessing, using and disclosing patient information relevant to the individual employee’s job duties. Also maintains confidentiality of other customer and hospital business affairs.

Adheres to the standards of Magruder Hospital’s corporate compliance policies and seeks guidance and training when necessary to resolve questions about the policies. Reports violations in accordance with corporate compliance and organizational standards.

Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.

Other duties as assigned

Bachelors Degree in either dietetics or an Academy of Nutrition and Dietetics (AND) approved program with completion of a dietetic internship; or pre-professional field work experience according to AND guidelines.

Computer experience, typing and a basic knowledge of Microsoft Excel, Word documents, PowerPoint and other software applications, EMR (Electronic Medical Record), and other equipment experience such as Fax machines, Copy Machines, Audio/Visual Equipment, etc. (These can be gained through training)

Ability to work independently or as a group

Ability to apply and/or research standards of nutritional care according to current protocols/interventions from recognized and reliable resources in order to complete the nutrition care process and develop nutrition education materials

To effectively provide nutrition education to patients, healthcare team, hospital personnel, and community

Serves as a consultant on nutritional care.

Ability to apply quality interviewing techniques for good patient rapport and involvement in their nutrition care management.
Preferred:

Serves as a consultant on nutritional care.

2-3 years experience in clinical nutrition and care of persons with chronic disease.

Food service background is also helpful but not necessary.

To have or obtain an NPI (National Provider Identifier)
-
Basic knowledge in Medical Record Coding for Diagnosis (ICD- International Classification of Diseases)

Basic Understanding of insurance coverage and reimbursement of services for Medical Nutrition Therapy and Diabetes or any other eligible coverage of services provided

Ability to development and modify recipes

Basic cooking skills and ability to teach clients those basic skills
License and Certification Requirements

Successful completion of the registration examination of dietetics and maintenance of continuing education to be registered by the Commission on Dietetic Registration (CDR) and licensed by the Ohio Board of Dietetics
