JOB SUMMARY:
The Independent Living Supports Pilot (ILSP) program provides case management services to older adults or disabled individuals who are in need of short-term care services that allow them to continue living independently in their own homes. Case Managers work with an assigned caseload of clients to develop individualized case plans and connect to needed flexible services or support, such as: specialized medical equipment and supplies, personal care, assistive technology, supportive home care, home and vehicle modifications, meals, personal emergency response systems, respite, non-medical and medical transportation, and more.

KEY RESPONSIBILITIES OF THE DISABILITY SUPPORT CASE MANAGER:
LOCATION: Office is based at the Kenosha Job Center. Local travel may be required to meet with clients within the home or other community spaces. Must be able to work on-site in Kenosha, Wisconsin.
SCHEDULE: Monday-Friday 8:00am-5:00pm with occasional evenings
